Appointments are recommended to help us prepare for your treatment and minimize wait times; however, walk-ins are always welcome.

If you arrive without an appointment, we will assist you as soon as possible based on provider availability.

Please plan to arrive 5–10 minutes early to complete intake forms and, if receiving IV therapy, select your desired cocktail.

If you arrive late, we will do our best to honor your scheduled appointment.

However, if your arrival interferes with the next client’s appointment time, your visit may be converted to a walk-in. Scheduled appointments will resume priority, and you will be seen as quickly as possible.

Hormone pellet procedures require advance preparation and reserved provider time.

 

• Please cancel or reschedule by the end of the business day prior to your appointment

• Late cancellations or no-shows may result in a $50 fee

Mobile appointments require staffing, preparation, and travel coordination.

• Late cancellations or no-shows will result in the mobile travel fee being charged

For IV therapy, injections, aesthetics, and general wellness appointments:

• Please notify us as soon as possible if you need to cancel or reschedule

• While fees are generally not applied, repeat late cancellations or no-shows may be subject to a fee at our discretion

• Memberships are billed monthly and may be canceled with 30 days’ notice

• Unused services roll over month-to-month as long as the membership remains active

• Unused services are forfeited upon cancellation

• Membership benefits are non-transferable

• Monthly fees are charged on the 1st of each month and are non-refundable once processed

• Payment is due at the time of service

• We accept major debit/credit cards and HSA/FSA cards for eligible services

• Due to clinical preparation, medications, and provider time, all sales are final and non-refundable

The Med Bar reserves the right to decline or postpone treatment if it is determined to be unsafe based on:

​

• Medical history

• Reported symptoms

• Abnormal vital signs

• Lab results

This ensures all services are provided responsibly and in your best interest.

We are committed to protecting your personal and medical information.

All records are securely stored within our HIPAA-compliant electronic medical record system (Boulevard).

• With your permission

• When required to coordinate your care

• As legally mandated

From time to time, photos or videos may be taken inside The Med Bar for marketing or social media purposes.

Your image will only be used with your explicit written consent. You may opt in or out at any time through our standard consent form.
